How many times have the 'Dream' or 'Nightmare' endings been done?

For the 'Nightmare' ending, I can only think of:
Igor (July 2007)
Mark (October 2008)
Gillian (December 2009)
Laurence (June 2010)
and, in the proveout:
Bill (November 2010)

And, for the 'Dream' ending, the ones that come to my mind are:
Sara (2005)
Donna (December 2006)
Gaz (early 2007)
Sylvia? (Christmas Stars 2007)
Louise (July 2008)
Jodie (June 2010)
Jenna (January 2011)
Suzanne (the only time in LIVE play)
Lauren (September 2011)
Pauline (July 2012)

Where are we up to with this now? I can only think of Loreen having the Nightmare ending and Shona the Dream ending, both in the proveout. Anyone I forget?
